I usually advise gals with oily/combination skin to follow these three rules: exfoliate, anti-inflammatory, and antibacterial. To keep your blackheads at bay, find a face wash with scrubby beads in it, or use a face loofah (you can get em super cheap at Sally's). Then make sure you use a toner with aloe, chamomile, cucumber, or some other type of anti-inflammatory property. Then for the antibacterial part, you can usually find a moisturizer with a natural antibacterial botanical in it, like tea tree oil, for example. DO NOT go get the benzoyl peroxide junk! It's super-harsh. I've had great luck with my clients who followed this routine with my products, and some of them had WAY worse problems than blackheads! Hope this helps a little....

Exfoliate 3 times a week... I do this at night when I'm on the shower after my workout... apply a ice cube after for tightinh your pores & plenty of moisturizer after.
I use St Ives apricot scrub... it seems to work very well & I have semi sensitive skin which is slightly combination.

I just saw this blackhead rememdy on Pinterest - I haven't tried it yet, but it looks cheap & easy:
BAKING SODA BLACKHEAD REMOVER
This gentle, quick facial uses baking soda’s acid-neutralizing alkalinity and milk’s lactic acid to clear out pores and lightly exfoliate.
1 tbsp baking soda
1/4 cup milk
Mix all the ingredients together. Rub into blackhead-prone areas using a soft makeup sponge. Rinse and repeat if needed.

Black heads are just pores with dirt/oil etc clogged in them. So the good news is that you can remove them, the bad news is that they are going to become clogged again. Facials usually help as do masques etc. However your going to have to come to terms with having them around (trust me most people do). I usually masque and exfoliate once a week and this keeps me black head free for about a week :)
I do oil cleansing method. I clean my face with cold pressed oils, castor, olive and grapeseed mixture with lavender, tea tree and ylang-ylang. I steam this off completely, I do this a few times a week, I also moisturize with facial oil. Alternate nights i wash with a glycolic acid wash.
